Forms and Petitions

Overview 

Students can use the online forms below to submit Cognitive Science specific forms (like major declaration, elective petition, or Honor's & independent study applications, etc.) as well as forms and petitions from other departments (like financial aid, study abroad, L&S advising, readmission, etc.

CogSci Specific Forms

CogSci Declaration Form

Students who are undeclared and have fulfilled all their prerequisites and are eligible for the major can submit their request to declare here. 

CogSci APR Correction Request Form

Is there an error on your APR (i.e. classes for one category showing up in another one)? You can use this form to request an advisor fix it! Note: CogSci advisors can only correct the Cognitive Science Major or CogSci & the  Future of Tech sections of your APR. 

CogSci Elective Petition Form

Any CogSci major who wants to use a course other than those listed on the Comprehensive Cogsci Course List as their upper division major elective requirement can request course review by submitting this form. These can be courses from any institution or Study Abroad program. Note: we do NOT accept petitions for the 6 Distribution Groups. 

CogSci 197 Academic Internship Credit Form

Have an internship? Want academic units for the internship? Submit the application using this form. 

CogSci 199 Research Credit Form

Working in a lab? Doing an independent research project with a faculty advisor? Want academic units for completing that project? Submit the application using this form. 

CogSci Honor's Program Application 

Want to do a CogSci Honor's project? Use this form to submit the application after you have gotten your faculty advisor's signature on this PDF.

Signature Request Form

Any time a student finds themselves with a form that requires a major advisor's signature, they should submit it to CogSci Advising using our Signature Request Form. This includes, but is not limited to the following:

  • Minor Completion 
  • Double Major Applications (for declared CogSci students)
  • UCEAP Academic Plans
  • General Program Plans
  • NCAA Eligibility 
  • Readmission Packets
  • Conditions to Declare 
  • Completion of L&S Minor Form
  • Financial Aid forms (including SAP appeals)
