Students who wish to add the minor in Cognitive Science and the Future of Tech must complete this L&S form, obtain the signature of the major advisor, and then submit it to the CogSci advising office using the CogSci Signature Request Form. Our office will then add the minor to your official record once the form is complete and submitted. Please note that the minor must be declared prior to the first day of classes in the student's Estimated Graduation Term. The minor courses do NOT need to be complete at the time this form is submitted. Also, while the minor will be added to your administrative record upon declaration, it will not be officially conferred until all required coursework is successfully completed.
