Starting Summer 22, CogSci will require most signature requests to be made via the CogSci Signature Request Google Form. This form can be used for:

• Minor Completion 
• Double Major Applications (for declared CogSci students)
• UCEAP Academic Plans
• General Program Plans
• NCAA Eligibility 
• Readmission Packets
• Conditions to Declare
